Easing the transition after discharge - helping to ensure excellent care
It is a sad fact that roughly 20% of patients discharged after treatment for certain diseases are re-admitted within 30 days. Readmission rates are directly related to a breakdown in post-discharge care, including not seeing a Primary Care Physician, patients not adhering to their post-discharge instructions and, often, the lack of an adequate support network.
By reaching out to these patients through a focused, proactive program of communication, we can help ensure that post-discharge instructions are being followed, medicines have been obtained and are being taken, and follow-up visits to healthcare providers are taking place.
In so doing, the patient’s post-discharge care and disposition are improved, and many readmissions are avoided before they occur.
